Free printable blood sugar log
Two blood sugar (glucose) log sheets to print and fill in by hand. Free PDFs, no sign-up, no email.
Which log sheet should I print?
- Weekly log (landscape): one row per day, with columns for before and after breakfast, lunch and dinner, and bedtime. Good if you test several times a day.
- Large print: one reading per row, with big boxes for the date, time, whether it was before or after a meal, and the reading. Good if you test once or twice a day, or prefer bigger text.
Both work with either unit. Tick the unit your meter uses at the top: mg/dL (common in the United States) or mmol/L (common in the UK, Canada, Australia and Europe).
How to fill in your log
- Write the number exactly as your meter shows it, and the time.
- Note whether it was before or after a meal. If your care team asks for readings after meals, ask them how long after.
- Use the notes column for anything that might help your care team read the numbers: what you ate, exercise, illness, missed or changed medicines, or a low reading you treated.
- Bring the log, and your meter, to every appointment.
When to test and what your targets are is different for each person, so this sheet has no target ranges printed on it. Your doctor, nurse or diabetes educator will tell you yours. If your meter or care team uses a different schedule, write the time in the box instead of using the column headings.
Tracking blood pressure as well?
Many people who log blood sugar are also asked to keep an eye on their blood pressure. Print a blood pressure log too, or keep both in one place with the BP Notebook app, which logs blood glucose (with before and after meal tags), blood pressure, pulse, weight, oxygen saturation and temperature.
Questions
Can I share or copy these log sheets?
Yes. Print as many copies as you like and share them with family, patients or a care home. Please don't sell them.
Will they print on A4 paper?
Yes. They are made for US Letter, but they fit on A4 if your printer's "Fit to page" option is on.
Does BP Notebook tell me if my blood sugar is high or low?
No. BP Notebook stores your glucose readings, shows them on a chart and puts them in your PDF report, but it does not label them as high or low. Your care team sets your targets.
